QEEG Assessment: $425 - includes a one hour active assessment in office, individualized report of findings and personalized training plan recommendations

Neurofeedback Training Session: $135 -  approx. 45 minutes in office, may include micro current, PBN/Infrared Light and/or brain training

Package A:  $1775 - includes QEEG and 12 training sessions (2 training sessions free)

Package B: $3500 - includes initial QEEG and 25 training sessions

Insurance Accepted for Psychotherapy/Counseling services: Blue Cross Blue Shield Illinois PPO, Aetna PPO, Cigna PPO, United Healthcare PPO

Out of Network clients will be offered a superbill to submit to insurance
All clients are responsible for deductibles, copays, and co-insurance fees if they choose to use health insurance coverage. 

Colorado

MBN is considered out of network currently. Clients can be supplied with a superbill to submit to their insurance for reimbursement.

Payment is due on the date of service.  No services will be offered if there is a balance due. No Acceptions
